Licensed Practical Nurse, Medical Assistant, Or Certified Nursing Assistant - Seymour Clinic

Location: 405 Commercial St, Seymour, WI 54165

This position is open to a Licensed Practical Nurse (LPN), Medical Assistant (MA), or a Certified Nursing Assistant (CNA)

Shift Hours:

Part-Time 0.40 FTE (16 hours/week), Mon - Fri 8 AM - 5 PM

Valuable team member of our team-based care model that provides direct nursing care under the supervision of a registered nurse/provider treating patients of all ages. This important role involves providing advanced patient care, observe patients’ total health and wellbeing, assisting providers, and communicating instructions to patients regarding home medications, home-based care, and preventative lifestyle changes.

Responsible for care delivered under the direct supervision of the Provider/Registered Nurse. Records patient care given and reports to appropriate person with changes in patient’s condition; consults with RN/Provider when uncertain about care delivery; provides basic health instructions and reinforces teaching by other health team members.

Qualifications:

WI LPN license -OR- graduate of an accredited Medical Assistant program. If LPN, license required within 3 months of start date. If Medical Assistant, AAMA certified within 3 months of start date.
