Summary:

LICENSED NURSING ASSISTANT I : To provide quality direct care services for geriatric and/or psychiatrically impaired residents, including providing assistance in activities of living and maintaining a safe and clean environment.


LICENSED NURSING ASSISTANT II: Provide continuous direct care services to the residents of the NH Veterans Home. Performs paraprofessional work associated with the tasks of daily living including assisting in providing motivation, recreation and learning activities.


LICENSED NURSING ASSISTANT III: Supervise and provide direct care services for assigned long-term geriatric residents, including performing para-professional work such as counseling with residents and acting as liaison between NHVH and community.

MINIMUM QUALIFICATIONS :

LICENSED NURSING ASSISTANT I:

Education: High school diploma or high school equivalency credential.

Experience: No experience required.

License/Certification : Must possess and maintain a current license for Nursing Assistant as approved by the New Hampshire Board of Nursing. Must possess a valid driver's license if responsible to transport residents to appointments.


RECOMMENDED WORK TRAITS: Ability to maintain a rehabilitative attitude and relationship with residents. Ability to relate to residents and co-workers in a positive and empathic fashion. Ability to respond to supervision and work cooperatively with others in the discharge of care and treatment responsibilities. Ability to perform routine manual tasks and primary resident care services. Ability to participate as part of a larger treatment effort aimed at re-socializing and rehabilitating institutionalized residents. Ability and willingness to participate in in-service training program to develop skills and knowledge leading to enhanced effectiveness. Ability to read and write to meet documentation requirements. Must be willing to maintain appearance appropriate to assigned duties and responsibilities as determined by the agency appointing authority.



LICENSED NURSING ASSISTANT II:

Education: High school diploma or high school equivalency credential.

Experience: Two years of experience as a Licensed Nursing Assistant.

License/Certification: Must possess and maintain a current license for Nursing Assistant as approved by the New Hampshire Board of Nursing. Must possess a valid driver's license if responsible to transport residents to appointments. One additional labor grade is provided for Medication Nursing Assistant Certification. Employee certification is reviewed and renewed biennially, in accordance with Board of Nursing certification requirements.


SPECIAL REQUIREMENTS: For promotion from Licensed Nursing Assistant I, labor grade 9 to Licensed Nursing Assistant II, labor grade 11/12, employees must:
Meet the minimum qualification requirements of the Licensed Nursing Assistant II;
Receive satisfactory annual performance evaluations; and
Be recommended for promotion by supervisor(s).
May be required to work non-traditional work hours to meet the needs of the long-term care facility.


PREFERRED WORK TRAITS:

Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

Ability to use personal computer and related software application.

Ability to communicate effectively, both orally and in writing.

LICENSED NURSING ASSISTANT III:

Education : Associate's degree from recognized college or technical school with major study in the social sciences or human services; OR a high school diploma plus 18 credit hours in the field of social services or human services.

Experience: Two years of experience as a Licensed Nursing Assistant. Each additional year of approved work experience may be substituted for one year of required formal education.

License/Certification: Must possess and maintain a current license for Nursing Assistant as approved by the New Hampshire Board of Nursing. Valid driver's license required if responsible to transport residents to appointments.

SPECIAL REQUIREMENTS: For promotion from Licensed Nursing Assistant II, labor grade 11/12 to Licensed Nursing Assistant III, labor grade 14/15, employees must:
Satisfactory completion of in-service hours, within the specified time-frame, as required by Nursing Staff Development.
Meet the minimum qualification requirements of the Licensed Nursing Assistant III;
Receive satisfactory annual performance evaluations; and
Be recommended for promotion by supervisor(s).
May be required to work non-traditional work hours to meet the needs of the long-term care facility.

Responsibilities:

LICENSED NURSING ASSISTANT I:

Provides for the needs of residents, such as cleanliness, nourishment, emotional comfort and activities such as sensory simulation as directed by a nurse.

Provides basic nursing care taking vital signs and collecting specific specimens for routine laboratory examinations.

Completes daily assigned tasks such as clothing distribution, replenishing charts, emptying laundry hampers, and cleaning furniture.

Observes and interacts with residents, reports observations to a Licensed Nursing Assistant II or nurse to assist in care planning.

Documents in records all care rendered and describe behaviors and reactions of residents.

Promotes resident independence in meeting individual needs.

May act as preceptor for orientation of new staff as assigned by supervisor.

Utilizes principles of infection control and universal precautions to foster resident treatment, recovery and/or prevention of infection.

Prepares and assists with transferring residents to special functions, trips, consults, religious activities, and other places within and outside the facility.

Encourages and maintains an open communication system and positive interactions with residents, families, and guardians.

Attends and participates in in-service educational programs to broaden knowledge related to the position.

Participates in organizational meetings and committees.

May be required to drive residents to medical appointments.


LICENSED NURSING ASSISTANT II:

Exercises some independent judgment and resourcefulness in providing for the daily care of residents, resolving minor physical and emotional needs. Ensures that appropriate needs are known when area is outside of scope.

Ensures that treatment plans are followed, including vital signs, performing bedside care, and collecting specific samples for routine laboratory examinations as directed by a nurse.

Maintains basic records for treatment and unit management purposes including notes regarding resident activity and behavior, evaluation of specific activity results, and ordering non-medical supplies for unit operations.

Provides residents with basic information, assists in interpersonal relationships and facilitates the adjustment of residents to their living environment.

As directed by a nurse, assists in planning and providing for the daily needs of residents with ADL and minor treatment procedures.

Supervises residents in various groups for resident enjoyment and maintenance of ADL skills and current levels of functioning.

Attends in-service programs and team meetings to broaden knowledge and enhance growth of work development.

Participates in organizational meetings and committees; presents documentation pertaining to resident's level of participation and response when applicable.

May administer medications, if in possession of a Medication Nursing Assistant Certification, within the parameters set by New Hampshire Board of Nursing and New Hampshire Office of Professional Licensure & Certification.


LICENSED NURSING ASSISTANT III:

Accountable for providing direct care to residents and delegating documented therapies and treatments as assigned.

Performs specialized tasks as demonstrated by satisfactory competency testing and/or certification.

Contributes information regarding residents’ needs and participates in setting goals/objectives in order to provide the most beneficial programming tailored to meet resident needs.

Plans, coordinates and implements groups and activities that correspond to the individual goals and objectives of the residents.

Documents resident response and recommendations for treatment.

Participates in organizational meetings and committees; presents documentation pertaining to resident's level of participation and response when applicable.

Leads and organizes activities relating to the maintenance of resident living areas and other primary resident care needs.

Assists treatment teams, with supplemental training received from discipline advisors, in achieving treatment objectives at the unit level.

Supervises, provides training and monitors activities of Nursing Assistants during shifts and as assigned.

Participates in and supervises specialized treatment activities including but not limited to recreation therapy, activity therapy and remotivation programs.

Attends in-service programs and team meetings to broaden knowledge and enhance growth of work development.

May administer medications, if in possession of a Medication Nursing Assistant Certification, within the parameters set by New Hampshire Board of Nursing and New Hampshire Office of Professional Licensure & Certification.

Perform all characteristic duties and responsibilities outlined by the class of Licensed Nursing Assistant II as assigned.
