As our Junior/Assistant Care Coordinator you will work with our senior care coordinators, marketing team and office personnel where assistance is needed. You will have the responsibility for marketing and maintaining accounts to learn the agency's protocols and standards for driving exceptional business. In this role you will have the ability to grow into a Care Coordinator position within a 6-to-12-month time frame and take full initiative to learn the Home Healthcare industry and business as well as the organization's vision, values, and expectations for success.


As part of our Dynamic Team Sales and Marketing Team you will:


  • Assist RDS and Senior Care Coordinator in any other task as defined by the supervisor
  • Demonstrates the company's Mission – Vision – Values through both behavior and job performance on a day-to-day basis.
  • Maintain detailed territory list, prioritizing accounts in accordance with the market/territory sales plan. Maintains detailed territory management systems records
  • Compile, communicate, and execute best practices in the field. Help develop new ideas, protocols, and efficiencies for maximizing marketing resources to grow market share.
  • Conduct pre-discharge hospital or other facility visits at the physicians' request and upon receiving physician orders to make a preliminary determination of the need/eligibility for home care.
  • Develop market-specific sales strategies to generate new business
  • Calls on physicians, hospitals, skilled nursing, and rehabilitation facility discharge planning services and management within an assigned territory to promote the sale of designated service lines.
  • Strive to learn and understand the organization's way of generating new business leads.
  • Document patient demographic, clinical, and payer information on company-approved forms and works with agency management to process referral


A Few Things You Will Need To Have


  • Flexibility with schedule and availability to attend after-hours events.
  • Ability to travel.
  • Possess excellent prioritization and time management skills.
  • Basic computer competencies with Excel, Word, and PowerPoint. Ability to learn and effectively use other business software programs
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
  • A GED or High School Diploma, Associates or Bachelor’s Degree (Years of Experience in a related field will supersede any advance education requirements).
  • Able to communicate effectively and clearly.
