Description


The Care Coordinator advocates on behalf of the client, making linkages/referrals for services, coordinate services and collaborating with other agencies for continuity of care for children exposed to sexual assault, domestic violence, dating violence and/or stalking and their non-abusing parent.


Essential Duties And Responsibilities


  • Attend and participate in project related training, interdisciplinary team meetings, staff meeting and community task forces
  • Coordinate services provided by ancillary agencies: linkages and referrals.
  • Understanding and application of federal, state and local confidentiality rules and professional standards of conduct
  • Perform other duties as assigned by the Director/Supervisor
  • Responsible for maintaining the clients chart in compliance with licensing, CARF, Medicaid, and other regulatory standards.
  • Conducts Needs Assessments
  • Prepares service plan documents
  • Provide direct services to those identified as children exposed to violence and/or their non-abusing parent
  • Provide sessions with respect to case management and Service Plan development
  • Development of the discharge plan in coordination with other treatment team members


Qualifications


Essential Qualifications:


Certifications/Licensures


  • Should be CAP, CPRS, or CPMH eligible. If not certified, must have ability to obtain the credential within 2 years of employmentPreference will be given to those candidates who are currently certified
  • Must be CPR/First Aid certified within 6 months of employment and every two years thereafter.


Education


  • Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required


Experience And Competencies


  • CPR Certification, First Aid Certification and an annual Tuberculosis test.
  • Ability to provide an Affidavit of Good Moral Character
  • Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
  • Ability to meet requirements of Monroe County Sheriffs Office background check
  • Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
