Primary responsibilities will be:

• Providing education to patients and family members regarding their home healthcare experience
• Serving as a point of contact throughout the admissions process
• Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers
• Providing an outstanding admissions experience for our patients
• Reviewing patient charts and ensuring information is clearly communicated to the patients care team
• Ensuring coordination of care with other team members

Must-have’s to be considered for this position:

• Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• Medical Assistant certification is a plus but not mandatory
• (Preferred but not required) Experience with HCHB EMR
• 1+ years' experience as an Intake Coordinator for SNF or Healthcare but experience as a Home Health Intake Coordinator is a plus
