We are currently seeking an experienced Healthcare Case Manager to join our team. The ideal candidate will be responsible for coordinating and providing care that is safe, timely, effective, efficient, patient-centered, and equitable for all patients.

Responsibilities:

• Facilitate communication and serve as a liaison between patients, family, healthcare providers, and insurance companies.
• Document patient progress and adapt care plans as necessary.
• Advocate for patient needs and coordinate services with other healthcare providers and organizations to address social and environmental health determinants.
• Collaborate with multidisciplinary team members to develop and implement care plans that ensure both short-term and long-term healthcare goals are met.
• Educate patients and their families on disease management, self-care, and wellness to enhance patient understanding and engagement in their own care.
• Assess, plan, and coordinate healthcare services for patients, working closely with the healthcare team to meet patient needs.
• Ensure compliance with healthcare regulations and safety standards.
• Manage entire patient treatment cycle, conducting regular patient evaluations to monitor and track patient's condition.

Qualifications:

• Knowledge of healthcare laws, regulations, and ethics.
• A bachelor's degree in nursing or related field is required; a master's degree is preferred.
• Minimum of 3 years of clinical nursing experience, with at least 2 years of case management experience preferred.
• Experience in using healthcare software and computer systems.
• Current, valid licensure as a Registered Nurse (RN).
• Excellent interpersonal, communication, and organizational skills.
• Certification in Case Management (CCM) is a plus.
• Ability to advocate for patients, make clinical decisions and coordinate with other healthcare professionals.
