Overview A Patient Case Manager is a healthcare professional who is responsible for coordinating care for patients with chronic illnesses or disabilities. They work with patients, families, and healthcare providers to ensure that the patient receives the best possible care. They may also provide support and guidance to patients and their families. Detailed Job Description A Patient Case Manager is responsible for coordinating care for patients with chronic illnesses or disabilities. They work with patients, families, and healthcare providers to ensure that the patient receives the best possible care. They may also provide support and guidance to patients and their families. They must be able to assess the patient’s needs and develop a plan of care that meets those needs. They must also be able to communicate effectively with all parties involved in the patient’s care. • Bachelor’s degree in Nursing, Social Work, or a related field
• Licensure or certification in a relevant field
