Job Description
This is a 12‐month ASHP‐accredited postgraduate year two (PGY2) ambulatory care pharmacy residency position that offers individualized training and experience. It is conducted in collaboration with CommUnityCare Health Centers and the University of Texas at Austin College of Pharmacy. This residency position will build on pharmacy education and PGY1 pharmacy residency experience to develop independent practitioners that deliver state of the art clinical pharmacy services to ambulatory care patients. Skills are developed through supervised, direct patient care in pharmacist‐managed clinics that provide comprehensive drug therapy management under a collaborative practice agreement approved by the CommUnityCare medical staff. The clinical pharmacist resident serves as an integral part of the multidisciplinary team and serves as the patient’s advocate in addressing their pharmaceutical care needs. This position will serve as a fundamental team member to continue to cultivate CommUnityCare’s team‐based care model initiatives.


ESSENTIAL / PRIMARY DUTIES

Manage the operation of an ambulatory care clinical pharmacy service under the direct supervision of ambulatory care clinical pharmacist preceptor


 Provide comprehensive medication management to ambulatory care patients as outlined in the Clinical Pharmacyprotocols/collaborative practice agreement


 Identify and resolve medication‐related problems


 Assist with medication refills as outlined in the Clinical Pharmacy protocols/collaborative practice agreement


 Provide health improvement, wellness, and medication education to patients


 Assume an active role in the education of students, residents, or other health care professionals


 Serve as a drug information resource for the health care teams


 Assist with adjudication of non‐formulary drug requests and act as a clinic resource regarding the formulary


 Assist with the maintenance of the collaborative practice agreement and protocols for clinical pharmacy services


 Develop a plan for a research project and a medication‐use evaluation


 Perform other duties as assigned


Knowledge/Skills/Abilities


 Extensive knowledge of medical/pharmaceutical terms and nomenclature


 High degree of knowledge of drug therapy


 Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures


 Develop and maintain favorable internal relationships, partnerships with co‐workers


 Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families


 Collaborate with all members of the care team in providing patient‐centered care


 Ensure all actions, job performance, personal conduct, and communications represent CommUnityCare in a highly professional manner at all times


 Uphold and ensure compliance and attention to all company policies and procedures as well as the overall mission and values of the organization


 Complete all mandatory training and maintain appropriate credentials/licensure


 High level of skill at building relationships and providing excellent customer service


 Computer systems knowledge, including but not limited to Microsoft Office products


 Strong attention to detail and accuracy


 Excellent verbal and written communication skills


 Ability to utilize drug information resources and communicate results written/verbally


 Demonstrated knowledge of local, state, and federal laws pertaining to Clinical Pharmacy activities as related to the use of drugs and other substances


MINIMUM EDUCATION


 Doctor of Pharmacy degree from an ACPE accredited college of pharmacy


MINIMUM EXPERIENCE


 ASHP accredited postgraduate year one (PGY‐1) pharmacy residency


PREFERRED EXPERIENCE


 Past preceptorship of pharmacy learners (Ex interns, residents)


REQUIRED CERTIFICATIONS/LICENSURE Holds and maintains these certifications as a professional. Lapsing/expiration of these certifications/licensure will result in suspension of work.


  • Pharmacist license with the Texas State Board of Pharmacy
  • BLS/ACLS within 3 months of hire
  • Completion of an ASHP accredited postgraduate year one (PGY‐1) pharmacy residency


REQUIRED COURSES/COMPLETIONS (e.g., CPR)


Basic Life Support (BLS) ‐ Obtained through approved American Heart Association Training Network or the Military Training Network.
