Job Description
LifeLong Medical Care is looking for a Supportive Housing Program (SHP) Case Manager to work at our Supportive Housing sites in Berkeley. The Supportive Housing Program supports the provision of integrated, long term, intensive services to homeless and formerly homeless adults with disabilities living in permanent supportive housing sites. Case Managers support and enhance the work of licensed clinical staff providing mental health and health care services. Major responsibilities include outreach, assessment, supporting housing retention, and providing and documenting intensive supportive services to formerly homeless clients in Permanent Supportive Housing.


This is a grant funded, full time, 40 hours/week, benefit eligible position.

Benefits

  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
  • Ability to understand and serve clients who have serious mental health, substance abuse, and unstable housing histories.
  • Be creative and mature with a “can do”, proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement.
  • Excellent writing skills, including ability to provide quality documentation of case management encounters.
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
  • Works closely with housing specialist partner organizations and maintains on-going relationship with landlords to support client housing stability.
  • Participates in regular case conference and team meetings.
  • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.
  • Conducts outreach to identify and engage residents in assigned Permanent Supportive Housing sites.
  • Coordinates closely with benefits advocacy staff to facilitate SSI application process.
  • Make appropriate use of knowledge/ expertise/ connections of other staff.
  • Ability to seek direction/approval from clinical supervisor on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Develops and maintains working relationships with outside social service agencies, ensuring that clients and staff have maximum access to community resources.
  • Commitment to the provision of health care and social services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
  • Documents all services provided in the required electronic data system and participates in clinical quality improvement review.
  • Works in partnership with LCSW to develop mental health treatment plans and assure interventions and services support mental health recovery.
  • Actively involves clients in the design and delivery of mental health and other recovery services by ensuring they have an active voice in their goal/service plans.
  • Maintains a caseload of 25-30 clients and provides intensive support services.


Job Requirements


  • Two years providing case management/social services/outreach including harm reduction and motivational interviewing techniques.
  • Proficient in Microsoft office.
  • High school diploma or GED.
  • Two years conducting outreach and engagement activities.


Job Preferences


  • BA in social work, psychology or related field or three years relevant experience preferred.
