We are a Federally Qualified Health Center (FQHC) and have been providing care to Yolo County residents since 1972. Like other community health centers, we provide care to anyone that walks through our doors and are located in all major cities within Yolo County (Davis, Woodland and West Sacramento) and provide services throughout Yolo County. The community health center movement was born out of the civil rights movement in the 1960s with the mission to provide healthcare in communities where there are few or no options for care. Salary:

  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
  • The pay range for this position is $22.75/hr.-$32.05/hr. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.

About the role of the Case Manager:

The Case Manager is part of a multidisciplinary team consisting of mental health clinicians, psychiatrists, Yolo County Juvenile Probation and supports identified within the Child and Family Team. The Case Manager utilizes a client-centered approach and collaborates with each person on their unique stability, resource and wellness needs in consultation with the Child and Family Team. The Case Manager demonstrates an ability to engage in trauma-informed and responsive service delivery and uses and models destigmatizing and strength-based language with clients and other team members. The Case Manager will be responsible for the facilitation of Child and Family Team meetings and work to link clients to critical resources, benefits, and supports.

Essential Functions:

  • Coordinates, schedules, and facilitates Child and Family Team meetings involving youth, their families, and formal and natural supports existing in their lives to better support their overall wellness.
  • Responsible for scheduling appointments and proactively outreaching in person or by phone/video.
  • Demonstrates ability to effectively work with patients and colleagues of diverse races, ethnicities, religions, affiliations, abilities, and LGBTQIA+ identities and life experiences.
  • Manages a fast-paced workday and can meet productivity requirements for program.
  • Completes case documentation within 48 hours of services in accordance with working hours, all documentation must comply with Medi-Cal, Beacon Health guidelines. Participate in peer chart reviews and other quality improvement efforts.
  • Provides case-management services within the Youth Services team for probation involved youth and their families.
  • Provides regular status updates regarding client appointments and other coordination issues, within limits of confidentiality, to Yolo County Juvenile Probation.
  • Participates in staff and departmental meetings.
  • Participates in team-based approach to care including staff meetings, case review, consultation/supervision sessions and other forums as required.
  • Attend trainings/conferences as arranged.
  • Understands the difference between Clinical and Case Manager role. Case Manager seeks consultation regularly and acts within scope of role.
  • Develops an understanding of CommuniCare, Yolo County, and community partners and builds effective working relationships.
  • Coordinates care and ensures linkage to referrals and troubleshoots barriers. Coordination is expected to occur while youth could be in custody, transitioning out of custody and in the community.
  • Responsible for remaining within established ethical standards, including those of confidentiality and mandated reporting.
  • As safe and appropriate to do so, transport clients or support with transportation arrangements.
  • Other duties as assigned.
  • Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and patient service standards.

Education/Experience Required:

  • Experience with youth and families strongly preferred.
  • 2 years working with those who are experiencing a range of mental and physical health conditions including those whose conditions may be severe and/or experiencing substance use disorders strongly preferred.
  • Bilingual preferred. Bilingual English/Spanish strongly preferred.

Ability to operate office equipment (telephone, copies, fax and computer), ability to bend, lift and carry up to 25 pounds, hearing required.

Special Skills/Equipment: Computer skills to include Microsoft Office and with training proficiently utilize electronic health record and data tracking software. Must provide current drivers license and auto insurance, ability to travel in Yolo County (West Sacramento, Davis, Woodland). Able to proficiently engage and provide services to patients by way of HIPAA-compliant telehealth/video-based services and fully participate in video-based meetings and trainings. Must have ability to effectively use telephonic translation services when client language preference dictates.
