  Job Summary:

  The Care Operations Specialist (COS) provides administrative support and management of client program scheduling. The COS works collaboratively with, and serves as a liaison to, the multiple internal departments and external clients supported by the Care Operations team.

  Major Areas of Responsibilities:

  • Create, coordinate, report and post all functions related to schedules for both patients and multidisciplinary team members both weekly and in the moment.
  • Monitor clients' meal logging during business hours and coordinate follow up with care team members.
  • Perform chart audits on a regular basis & follow up with appropriate care team members to ensure completion.
  • Update and track new admissions, discharges, and changes in level of care in a centralized matrix.

Pay range: 20-$24/Hour
