Aging Care Coordinator
Title: Aging Care Coordinator
Job Classification: Non-exempt
Employment Category: Part-time / Per diem
Supervisor: Director of Aging Services
The Aging Care Coordinator supports clients and their families by providing comprehensive assessments, developing care plans, monitoring ongoing care, managing the Therapeutic Companion, communicating with and coaching the family, and coordinating the interdisciplinary team.
The Aging Care Coordinator is the point person for the family and will act as the liaison between the client, family, and providers to ensure effective care. Aging Care Coordinator will meet clients in the community, remotely, or in the home setting to perform actions necessary to meet their needs and goals.
Compensation:
Bachelor’s Degree - $25-$45
Master’s Degree - $45-$65
PhD, PsyD - $65-$125
Job Responsibilities:
- Reviewing clinical history and contacting providers
- Supervising Therapeutic Companions and working collaboratively with other OPG team members
- Providing consulting services to the client and family members around aging
- Completing initial evaluations and comprehensive assessments of the client and their environment that are done both in-person and virtual
- Conducting family meetings to improve family dynamics
- Conducting psycho-social evaluations and assessing cognitive abilities and mood
- Traveling throughout the area for evaluations, home visits, and appointments
- Participating in company supervision and completing required company education materials
- Assisting clients in moving into senior living communities
- Providing coaching services to identified individuals
- Developing a care plan with the client and identified supports
- Documenting all client interactions, outcomes, and progress toward goals
- Having a basic understanding of care management
- Communicating with outside treatment providers and coordinating care
- Maintaining communication with client contacts, including family, physicians, attorneys, financial offices, and other care supports
- Having a basic understanding of the differences of Medicare, Medicaid, Private Insurance, and Long-Term Care policies
- Advocating for clients based on individual medical, financial, and social needs
Additional Responsibilities:
- Emergency support, on-call availability, and client transportation, as needed
- Additional duties will be assigned as deemed appropriate
Qualifications:
- Experience working closely with families & professional providers
- 4-5 years of experience in care management, home care, acute care, and/or a rehabilitation setting
- RN, LSW, LCSW, LICSW, OT, or PT in good standing preferred; Case Manager or Care Advocacy in the elder field with 5+ years of experience also considered
- Excellent written and oral communication skills
- Experience with elder care
- Experience in behavioral health preferred
