Community Based Care Manager Jobs

Full-time – 40 hours/week - $65,000 - $75,000 annually depending on qualifications and experience

A minimum of 5 years of experience in leadership in an organization that does community-based work

At least 10 years of experience in professional leadership positions of any type

Experience and competence in teaching and training

Experience in coaching other leaders to improve their ability to lead their organization into healthier community engagement

Experience in building coalitions from diverse sectors to engage in collaborative work in their communities

Responsible for training, performance management, and record management.

Perform any other job-relate duties assigned by their supervisor or management.

Minimum 2 years related business intelligence experience and/or training; or equivalent combination of education and experience

Minimum 2 years working experience in international hospitality and/or travel industry

Strong Excel skills required. Ability to analyze large datasets and report relevant finding. High attention to details

Reviews process implementations and comes up with recommendation for continuous improvement

Become a primary support in helping children/youth and families to develop preventive and responsive skills to use during crisis situations.

Certification in an Evidence-Based Practice consistent with the Community-Based Support Counselor components being delivered. OR

One year of applicable experience. OR

Certification in an Evidence-Based Practice.

Registered Professional Nurses with one year of behavioral health experience.

Experience working with children and families in a community-based or "in-home" setting.

Change management experience is preferred.

Experience empowering each hospital role to the full extent of their skillsets is required.

Three years of experience practicing as a Doctor of Veterinary Medicine or leading veterinary teams are required.

Veterinary hospital leadership experience is required.

Minimum 5 years creative copywriting experience. Apparel or retail industry preferred

Experience with integrated, multichannel brand work (video, print, OOH, social, experiential, paid digital etc.)

Knowledge of industry standards and copy best practices

Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;

Maintain professional memberships, licenses, accreditation, certifications, etc.;

Proficient computer skills, specifically with all Microsoft office applications;

Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;

Effective communication skills, organizational skills, and ability to multi-task.

Responsible for value-based care program maintenance, including things like provider roster management, government program database entry, and tactical performance.

Develop tactical plans to ensure consistent deployment of ACO REACH education and program implementation across markets.

Manage and facilitate periodic reports, as well as any necessary ad hoc reports, to provide insight into REACH performance.

Experience from the payor and provider side preferred, one required.

Experience designing and operating multiple VBC arrangement types (bundles, capitation, shared savings/risk) preferred
