Social Worker - Independent Living - Full Time Position

SUMMARY The Independent Living Social Worker is responsible for evaluating the independent living needs of residents, assisting with the development of programs to meet those needs, teaching self help techniques to ensure that the medical, social, and emotional needs and the general welfare of residents is met, monitoring and directing level of care transitions/needs. Participates in the decision-making process regarding resident admissions to the Health Care Center. Assists residents and their families with personal and environmental difficulties that predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Maintains communication with physicians, families and appropriate staff regarding resident status.
  • Evaluates the physical, emotional and/or social needs for supervision of care.
  • Documents the resident's social and health adjustments.
  • Provides service planned to restore resident to optimum social and health adjustment within patient's capacity.
  • Conducts/coordinates resident educational and support groups.
  • Maintains or improves independence through coordination of medical staff/therapy departments in the use of appropriate adaptive equipment or techniques.
  • Assists in interpreting various policies/procedures for the residents.
  • Assesses living environment and recommends adaptation to eliminate unsafe/health hazards.
  • Participates in planning for improving health services by interpreting social factors pertinent to development of program.
  • Must interact courteously, tactfully and politely with managers, co-workers, residents, vendors or associates to preserve productivity and moral.
  • Assists residents and families by offering support in dealing emotionally with the aging process and illness.
  • Assists in coordinating services for residents being admitted to other levels of care.
  • Helps resident and family through individual or group conferences to understand, accept, and follow medical recommendations.
  • Utilizes community resources to assist resident to resume life in community or to learn to live within limits of disability.
  • Conducts assessments of IL Residents to determine level care and placement with an interdisciplinary team.
  • Assists and encourages residents to participate in social and creative activities.
  • Assists in the monthly orientation of all new residents to adjust to community living.
  • Participates in ERT Committee.
Qualification:
Bachelor's Degree in Social Work or Human Services related degree is required. Experience working with seniors is preferred.

Work Schedule
Full-Time – 8:00am - 5:00pm Monday - Friday. Some evenings and weekends may be required to meet the demands of the position.
