Social Worker - Hiv - Remote | Wfh

We are currently seeking a highly motivated and experienced Clinical Social Worker to join our team. As a Clinical Social Worker, you will collaborate with multidisciplinary teams, provide psychotherapy, coordinate patient discharge planning, and serve as a resource to other allied and administrative staff within the section of Social Work and the Department of Nursing.
Responsibilities
Collaborate with multidisciplinary teams to provide comprehensive care to patients.
Provide psychotherapy to patients and their families.
Coordinate patient discharge planning and ensure that all required medical/legal standards are met.
Serve as a resource to other allied and administrative staff within the section of Social Work and the Department of Nursing.
Qualifications
A current licensure at the clinical level and eligibility to practice independently as a Social Worker.
An MSW degree from an accredited Social Work program.
Advanced knowledge of mental illness, personality development, and the types of treatment.
Skills in conducting individual, group, and family therapy.
Knowledge of adjustment to medical diagnosis’ and chronic disease.
Strong communication skills to interact effectively with patients, patient’s family members, multi-disciplinary team, community agencies, and other resources.
Psychosocial diagnostic skills and the ability to communicate effectively in both verbal and written form.
Valid driver's license required for specific specialty areas.
