Job Title: Social Worker – Emergency Department


Social Work Services at the Mount Sinai Hospital


The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.


  • The social worker in this position will facilitate the discharge of patients in the ED/ RETU.


Responsibilities

  • The ED/ RETU social work team also provides coverage for urgent needs that come up after-hours across the hospital.
  • Services will include counseling; crisis intervention (including cases of domestic violence, child abuse and neglect, sexual assault, human trafficking, and death), assistance with concrete services (e.g., pharmacy, home care, transportation, entitlements, referral for community services), placement of patients in facilities and, close collaboration with social workers in outpatient settings and Care Coordination programs to whom patients may be referred for follow-up.
  • Responsibilities include psychosocial assessment; a full range of psychosocial interventions for patients/families; transitional care; and, when appropriate, follow-up phone calls; and post-discharge intervention.


This full-time position requires THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month. The schedule will include weekend days on a rotating schedule. The hours are 7 AM – 7:30 PM. Schedules are subject to change based on ED/RETU patient flow.


Qualifications

  • Bilingual (English/Spanish) preferred.
  • Exceptional skills in engaging patients and families and expertise in coordinating resources within the hospital and community networks for effective management of patient care.
  • Strong interest in being part of innovative initiatives to improve the quality of care for patients who present in ED/RETU.
  • Excellent clinical skills, with exceptional skills in rapid psychosocial assessment and focused short-term treatment.
  • Strong interdisciplinary/collaborative skills and organizational skills.
  • Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.


Compensation Statement


The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $82,000.00 - $94,000.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
