Job Title: Social Work Fellow - MSBI
Roles & Responsibilities:
  • Participates in Divisions Quality Improvement activities.
  • Insures bereavement resources are provided for families and initiates referrals, when appropriate for high risk families.
  • Participates in data collection.
  • Facilitates communication with patient/family with regards to advance care planning.
  • Contributes to assessment of pain and symptoms to foster multidimensional treatment plan.
  • Documents psycho-social assessment and plan on palliative social work notes in electronic medical records.
  • Performs other duties as directed by Social Work Coordinator or Chief of Service.
  • Insures completion of the IDT form and contributes the psycho-social assessment and plan.
  • Assists the team to facilitate patient/family meetings in clarifying goals and plan of care with attention to family dynamics.
  • Attends didactic, academic and weekly administrative palliative care meetings.
  • In collaboration with primary social workers, assists patients and families with community support services, post hospital plans, financial and/or insurance issues, anticipating long terms needs to enhance continuity along illness trajectory.
  • Identifies and addresses ethical and legal issues related to patient and family care.
  • Enhances interactions with the patient and the bereaved family by encouraging communication of clinical concerns and enriching end-of-life experience that support and inform post death legacy.
  • Participates in daily palliative care rounds.
  • Assesses risk and intervenes clinically to identify potential for complicated grief and explores anticipatory bereavement related to possible death and multiple aspects of loss and adaptation along the continuum of illness.
  • Provides psychosocial assessment and interventions to patient/family in collaboration with palliative consultation and primary teams.
  • Prepares family for end of life, emotionally, cognitively and with practical tasks and needs.
Requirements:
Education Required
Masters of Social Work
Licenses / Certification
Permit required. LMSW to be obtained within the year.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $59,885.00 - $89,843.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
