DAY-TO-DAY

This full-time case management position in human services works during the day.

As a Social Work Assessment Coordinator with our nonprofit, you develop, implement, and deliver various assessments to determine current and potential barriers affecting our participants. You also identify transferable skills and experience, career aptitude, motivation, and education level. By ensuring each assessment is tailored to the individual, you use the results to develop individualized service plans that address long- and short-term goals for our participants. You work hard to establish honest and effective relationships with participants to help them reach their goals.

Additionally, you evaluate the effectiveness of each assessment and its implementation. You also conduct thorough interviews with our applicants as well as facilitate the orientation and intake processes. Using your exceptional time management skills, you ensure our participants undergo a smooth transition and short wait time for our program. Requirements For a Social Work Assessment Coordinator

• 2+ years of case management or assessment experience
• Experience working with public assistance recipients, the previously incarcerated, and the economically disadvantaged
• High school diploma or equivalent
