General Summary of Position
Responsible for the daily leadership and clinical activities of the MedStar Montgomery Center for Successful Aging, Geriatric Outpatient Clinic and the Geriatric High Intensity Outpatient Clinic. This includes ensuring the highest quality of care, patient satisfaction and regulatory compliance. Responsible for the development and implementation of clinical workflows and activities related to the Center for Successful Aging, MedStar Montgomery Medical Center. This includes oversight to clinical support staff responsible for the implementation of routine geriatric primary care, case management, and clinical coordination. Works with MMMC Nursing and other staff to develop and implement referral standards, transitions planning and other clinical standards to ensure the safety and well-being of Geriatric Patients. In coordination with the Center for Successful Aging Administrator develop support staff roles, responsibilities, and measures of success and provide supervisor roles in evaluation of the standards. Assists the Administrator for the Center for Successful Aging to optimize efficiencies and develop performance improvement initiatives in support of MMMC strategic, operational, and financial goals. Work with senior leadership and physicians of MMMC and MedStar Health to support growth, collaboration, and partnerships, in accordance with strategic and operational plans. Ensure that clinic support staff adheres to the MedStar S.P.I.R.I.T. values in all activities.
Minimum Qualifications
Education
- Bachelor's degree in Nursing required
- Master's degree preferred
Experience
- Geriatric nursing experience preferred
- Rn supervisory experience in a medical office or ambulatory health setting required and
- 3-4 years Progressively more responsible job related nursing and medical office practice experience, preferably in a busy multiple physician practice and solve patient and staff problems required
Licenses and Certifications
- CPR certification required and
- Valid RN license in Maryland or Compact State required
Knowledge, Skills, and Abilities
- Must be able to work effectively among all levels of staff and internal/ external customers.
- Excellent Written and Oral communication skills.
- Excellent Customer Service/ patient interaction skills.
- High-level problem-solving skills as well as independent judgment skills relating to clinical care policy and procedures.
- Knowledge of Joint Commission standards required.
- Must demonstrate effective communication skills.
- Excellent leadership and managerial skills.
- Knowledge of NICHE and/ or IHI Age-Friendly Health System standards preferred.
Primary Duties and Responsibilities
- F. Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (CPR, License, PPD) g. Meets all mandatory obligations. h. Maintains continuing education program for staff by giving or arranging for in-services and providing for training opportunities.
- E. Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit. f. Supports the CSA Administrator regarding MSH Philanthropy team to nurture current and potential ongoing philanthropic requests. g. Integrates the CSA model of care with current NICHE Standards and work with nursing and physician leadership to develop towards the IHI Age Friendly Health System.
- Center for Successful Aging (CSA) Daily Activity Management: Develops and implements the operational and clinical standards for the CSA, consistent with MMMC Quality and Standards of Care. a. Adapts and/ or develops patient clinical workflows and procedures to ensure a positive patient experience consistent with the MedStar Good Samaritan Hospital (MGSH), CSA Model of Care b. Works with MMMC Nursing and Physician leadership to develop referral standards, within the MMMC to ensure appropriate transitions of patients between relative work areas.
- C. Ensures that workflows and procedures comply with local MMMC standards of care, The Joint Commission, Nurses for Improving the Healthcare System for Elders (NICHE), and the Institute for Healthcare Improvement (IHI) Age-Friendly Health System. d. In coordination with Quality, Risk Management, and other MMMC staff ensure that patient safety and risk assessments are built into workflows and identify patients that may have higher risk of hospital related injuries.
- B. Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees. c. Develops and maintains effective and collaborative relationships with physicians and other providers. d. Develops and fosters effective and collaborative relationships with MGSH CSA and Regional Geriatric Service Line Staff.
- C. Attends appropriate continuing education opportunities to maintain licensure and advance knowledge in Geriatric Nursing and Outpatient care standards. d. Participates in training classes for quality and safety, infection control, OSHA, EMR, CPR, TJC, safety, compliance and others as required. e. Attends hospital sponsored training programs as required by the department.
- B. Works with CSA Administrator to identify additional support requirements aligned with CSA and MMMC strategic and operational planning to include but not limited to: Community Health Workers, Case Managers, Dieticians, and Clinical Pharmacists. c. Develops and implements daily work performance expectations and ensures the daily clinical operations for the MMMC CSA. d. Provide routine supervisory responsibilities in the evaluation of performance outlined in the staff position requirements
- Organizational knowledge: Works effectively within and between departments a. Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate and community outreach efforts, as appropriate.
- E. Work with the CSA Administrator to coordinate workflows and support with MedStar Medical Management Group and Specialty Care providers for routine specialty support based on appropriate case load and support opportunities. f. Identifies supply standards and works with support staff for the routine management of supplies, to include but not limited to: Physician Office Supplies, appropriate medical supplies, maintenance of ACLS (Crash) Carts, and any pharmaceutical or other time sensitive clinical materials within the CSA.
- Development: Maintains current knowledge and develops new knowledge a. Practices within applicable scope of practice and according to hospital policy and procedures. b. Identifies learning needs in self and initiates actions to address these education needs, especially related to Geriatric Medicine.
- Supervisory Responsibility for CSA Clinical Support Staff: Provides daily supervision and accountability for CSA Clinical Support Staff, ensuring the appropriate provider support a. Develops and trains or coordinates for training of assigned clinical support staff to include Medical Assistants (Personal Services Coordinators), Registrar, and Social Worker (Case Management). .
