RN, Admission Coordinator - PRN

Remote Admission Support Nurse (RN)who works offsite to help support the nurses in the facility with the admission process. The Remote Admission Support Nurse will assess and improve admissions at the facilities.

Essential Functions

• Respond to inquiries in a timely and professional manner through various methods of communication. Ensure that potential customers receive the highest level of customer service.
• Must function independently, make independent decisions, and demonstrate flexibility, personal integrity, and ability to work effectively with residents and personnel.
• Consistently demonstrates (leads by example) Trinity Values to all internal and external customers (residents, visitors, volunteers, vendors, and associates). Creates and maintains an atmosphere of warmth, propagating a calm environment throughout the community.

Primary Functions:

• The admission tasks will be separated between an offsite nurse and a facility nurse.
• Admissions Nurse will enter their assigned tasks into the Electronic Medical Record.
• Admission Nurse will access EPIC/hospital EMR when possible An initial patient chart review may include Diagnosis,Orders,Hospital EMR- H&P of resident stay, labs, xrays
• Follows a Standardized Admission Process established by Remote Admissions Program. Will support nursing manager to complete a full admission process.
• Be a clinical resource for admission process and assessments. Supports multiple facilities in the admission process.

Minimum Qualifications

• Prefer recent nursing experience with the elderly in Skilled Nursing Facility or Rehabilitation.
• Must be a Registered Nurse with at least 5 years experience, Bachelors degree preferred or equivalent experience in Nursing. Willing to be licensed in state the THSC operates in.
