Performs active service recovery and appropriate escalation to management to address patient and staff concerns.

3+ years progressive nursing experience with demonstrated leadership skills in community health nursing.

Uses clinical knowledge and critical thinking to triage, assess, evaluate and implement interventions for urgent and nonurgent clinical patient needs.

Provides counsel and clinical education to patients, family, and caregivers.

PA, NJ, and DE RN licensure depending on geographic location and scope of responsibilities

Adheres to establish standards with work performance delineated at UPHS and prescribed by regulatory agencies.

Ensures patients and families have the best possible experiences across the continuum of care.

Demonstrates good communication and interpersonal skills in dealing with patients, medical and clinical staff, payers, service providers, and community agencies.

Cultivates relationships with physicians, nurse managers, discharge planners, other community agencies and internal facility programs to support referrals.

Demonstrates excellent organizational and prioritization skills. Demonstrates high initiative, flexibility, and ability to multi-task. Thrives in fast-paced environment.

Demonstrates responsible use of LRH's resources including people, finances, equipment and facilities.

Minimum of Associates degree in Nursing. Minimum of 1 year experience in rehabilitation healthcare or allied health.

Admissions/behavioral health call center knowledge and experience

Ensure knowledge of census and bed board for efficient admissions flow.

Coordination with UR and finance for admissions

Demonstrates consistent knowledge and competence of patient census and patient flow to ensure efficient access to care

1+ years of behavioral health admissions experience required

Clinical skill to coordinate with pts and families

Teach and coach therapeutic skills to unit staff

Excellent verbal and written communication skills

Minimum of three (3) years of experience working directly with children and adolescents in a psychiatric or residential setting preferred

Serve as designated leadership on shift

Twelve months of experience in data entry and/or word processing with demonstrated expertise in patient registration functions.

Ability to maintain high level of confidentiality and to prioritize multiple tasks and activities.

Interviews patients or responsible person to gather necessary billing, insurance, and medical record information; assigns patient number.

Verifies insurance and enters insurance, and medical record information into the Hospital computer system.

Prepares patient label and identification wristband.

Explains to the patient Hospital regulations concerning visitors, visiting hours.

Follows a Standardized Admission Process established by Remote Admissions Program. Will support nursing manager to complete a full admission process.

Prefer recent nursing experience with the elderly in Skilled Nursing Facility or Rehabilitation.

Must function independently, make independent decisions, and demonstrate flexibility, personal integrity, and ability to work effectively with residents and personnel.

Be a clinical resource for admission process and assessments. Supports multiple facilities in the admission process.

The admission tasks will be separated between an offsite nurse and a facility nurse.

Admissions Nurse will enter their assigned tasks into the Electronic Medical Record.

Seeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissions

Must possess excellent verbal and written communication skills.

Ensures appropriate pre-admission documentation is complete

Ensures accurate collection of referral, denial, and admission’s data through maintenance of an excel data file

Works with financial counselors to assist our uninsured with potential access to the rehab level of care

Ensures accurate collection of data related to the insurance authorization process for potential admissions through maintenance of an excel data file

Minimum of four years of directly related experience. (ie: Experience in Business Office customer service, Accounting, Healthcare Admissions/Registration.
