Works as a member of a multi-disciplinary team in assessing, developing, implementing and evaluating the nursing component of the individualized care plan for each client.
Meets regularly with nurse team leader for nursing standard of practice supervision, consultation and evaluation.
Performs nursing duties according to nursing and program standards.
Provides nursing care according to identified client and community needs.
Works collaboratively with the clinical providers at the primary care sites through participation in case conferences and in other informal communications.
Performs health education and training for clients during home visits, clinical site appointments or group settings in the community.
Develops individualized care plans/ family development plan and intervention strategies for each client.
Delivers services and documents nursing care according to nursing and program protocols.
Identifies emerging problems and solutions for clients and their communities.
Continues professional education to enhance knowledge, skill and ability to maintain a high level of clinical practice.
Performs other duties as required.
Minimum Qualifications
MINIMUM REQUIREMENTS
- Must be available to work flexible hours including weekends and evenings.
- Must be licensed RN in Massachusetts; BSN preferred.
- Bilingual fluency preferred.
- Experience working as part of a team of providers to deliver and coordinate care.
- Good organizational skills with the ability to develop time lines meet deadlines and be autonomous.
- Maternal / child health experience.
- Good interpersonal skills and ability to communicate effectively both verbally and in writing.
- Demonstrated cultural competence working with diverse ethnicities, cultures and socio economic groups.
- Previous community health experience including home visiting and knowledge of Boston communities.
