Main Areas Of Responsibilities

Collaborative Case Conferences: Working hand-in-hand with a dedicated team, you'll participate in case conferences to ensure comprehensive and holistic care plans for our members.
Post Hospital Care: After hospital visits, you'll follow up with our members, offering compassionate guidance and assistance during their recovery process.
Caring Home Visits: Put your interpersonal skills to work during in-home visits, providing personalized support within the comfort of our members' homes.
Health, Safety, and Welfare: You'll champion the well-being of all members in your caseload, ensuring their health and safety remain a top priority.
Requirements
