Job Description

Title: IMEP Facilitator Revision Date: 6/16/2023
Reports to: Director of Health Services
Department: Health Care Administration

JOB SUMMARY

Responsible for the implementation of all the components of the IMEP including activities, exercises, daily operating management and maintenance of all program and participant records.

ESSENTIAL DUTIES & RESPONSIBILTIES:

  • Work as a team with other staff and volunteers in presenting the daily program and activities.
  • Understand the needs of the elderly and in particular the needs of those affected by memory loss.
  • Ability to provide basic First Aid and when deemed necessary, call 911. Notify IMEP Manager after calling 911. Notify participant’s emergency contact after calling 911.
  • Be willing to work at other IMEP locations when needed.
  • Ensure that at least two staff or one staff and one trained volunteer are present during all hours of program operation.
  • Comply with all HIPAA privacy laws and all COA policies and procedures.
  • Perform related duties as assigned by supervisor.
  • Provide observation, monitoring and intervention if unstable mental status or medical condition arises.
  • Interact with, be sensitive to, and respond to the needs of participants and family caregivers.
  • Attend IMEP staff meetings as scheduled.
  • Contact IMEP Manager regarding any problems, questions, concerns, or emergency.
  • Monitor the provision of meals and participants dietary and fluid intake.
  • Adhere to the IMEP Daily Agenda including all therapeutic components, exercises, and activities as scheduled, unless group needs necessitate a schedule change.
  • Ensure that all program paperwork including sign-in sheets, time sheets, client information forms, and emergency contact information forms are recorded and accurate.
Qualifications

EDUCATION & EXPERIENCE:

  • Experience in the nursing field a plus.
  • Knowledge of early memory loss and early dementia.
  • Ability to work independently in an off-site location.
  • Experience in small group facilitation and/or education.
  • Be trained or be willing to be trained in CPR and First Aid.

Ability to safely and successfully perform the essential job functions consistent with meeting qualitative and/or measurable productivity standards. Ability to maintain regular, punctual attendance consistent with the COA policy. Must be able to operate cellphone and computer.Ability to stand for long periods of time. Must be able to listen and speak clearly.
