What You'll Do
As a Community Resource Coordinator, you will be at the heart of our community outreach efforts. Your days will be filled with making a difference, as you:
Collaborate with our healthcare team to identify patients in need of non-medical support.
Actively seek out solutions to address a wide range of challenges impacting health and well-being.
Engage and connect with patients in their homes, medical offices, or other community settings.
Assist patients with financial and insurance concerns, transportation needs, access to behavioral health services, in-home support, legal assistance, and more.
Requirements
We're looking for individuals who are compassionate, resourceful, and enthusiastic about community service. If you possess the following qualifications, we would love to hear from you:
A Bachelor's Degree in Social Work or a related Human Services Field.
Previous experience in the human services field, preferably in a community services or medical setting.
A reliable car, a current driver's license, and a safe driving record.
