Job Description
The ideal candidate will be responsible for reaching out to community resources (doctor offices, acute hospitals, SNFs, ALFs) for patient referrals.
Responsibilities include:
Strengthen our relationship with referral sources and company partnerships
Build new relationship with community resources
Track, analyze, follow up on community resources
Follow up on patient records and documentation
Create online and offline marketing content
