Medical Assistant/PATIENT CARE COORDINATOR II - Pelvic Health
Want to be part of a great team? The Pelvic Health Center is looking for a Patient Care Coordinator (PCC) to be part of our strong team environment. A PCC works as the point person navigating the patients from every discipline through their clinic visit in the Pelvic Health Center and Gynecology Clinic in Redwood City. We’re looking for someone who enjoys working with patients and helping them obtain the care they need. Our team members are cross trained to work in multiple roles from front desk to back office. The PCC rooms patients, chaperones during patient visits, checks patients out, schedules follow up appointments, sets up procedural areas, assists providers during procedures, manages providers schedules and ensures timely communication between the patient and the provider by responding to patient messages.
Patients with pelvic floor dysfunction are referred to this clinic to confirm diagnosis or for follow up treatment. Our patient population includes both men and women who have a variety of pelvic related medical concerns. We use a multidisciplinary approach to care which includes specialists from Urology, Gynecology, Colorectal, Gastroenterology, Physical Therapy and Pain.

A Brief Overview
The Patient Care Coordinator functions as a member of the patient care team. The position performs clinical tasks and procedures to support the delivery of care and to promote efficient use of the physician's time as well as performs all clerical duties required to complete patient Check-in/Check-out. The Patient Care Coordinator is responsible for assisting patients obtain services in a timely manner and assuring efficient utilization of the physician's appointment schedule and in the coordination of patient care. This position administers and coordinates referrals and service authorization process, rooming duties and performs other clinic front desk activities.

What You Will Do

  • Patient Care
  • Prepares the necessary information (i.e. lab and procedural results), equipment and supplies for exam.
  • Administers medication following approved procedures and only after checking medication and dose with a licensed person.
  • Effectively responds to inquiries from patients, their relatives and/or referring physicians on services and provides accurate information in a timely fashion.
  • Maintains clean and orderly exam rooms, hallways and work areas.
  • Promptly and accurately reports and refers patient’s concerns and symptoms, reactions, changes (appropriate to age) to licensed clinic personnel.
  • Assists physicians with procedure set-ups, patient procedures and examinations.
  • Processes requests for prescription refills according to established practice.
  • As required, performs waived testing (urine dipstick, urine pregnancy test, blood glucose testing), perform EKGs, administer immunizations.
  • Schedules and edits and maintains patient physician appointments for new and returning patients, using the scheduling system.

A Brief Overview
Nurse Coordinator describes a variety of specialized nursing positions that provide professional support, planning, coordinating, and facilitating patient care for selected patient populations across the continuum of care, ensuring compliance with provider orders and established policies and procedures. This position utilizes the nursing process without providing direct patient care. Nurse Coordinators provide coordination and care for patients requiring assistance and support in accessing and managing their health care needs. As an integral member of the care team, they collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services. Responsibilities include coordinating patient communication, patient education, and preparing patients and/or caregivers for procedures and tests in collaboration with the health care team; ensuring continuity of care and coordination, identifying opportunities to improve patient care services; and assisting clinic management to identify and develop operational needs.

  • Expedites appropriate care and treatment, using critical reasoning and astute clinical judgment especially given the patient may present with complex problems or potentially life-threatening conditions

  • Utilizes performance improvement methodology to design data collection systems / processes and identifies opportunities to improve care / service for patients.

Education Qualifications
  • Bachelor’s degree or equivalent from an accredited college/university Required
Experience Qualifications
  • Two (2) years of progressively responsible and directly related work experience

Base Pay Scale: Generally starting at $68.38 - $90.62 per hour
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
