Home Health Patient Care Coordinator Jobs

Maintains the client hospitalization log, including entering coordination notes, and sending electronic log to all office, field, and sales staff.

Completes requested schedules for all add-ons and applicable orders:

Schedules discharge visit / OASIS Collection or recert visit following case conference when task appears on action screen.

Checks in all SUD and mental health group participants in NextGen Patient Management.

Responsible for coordinating with case manager and provider ahead of SUD visit.

Serves as back up to Office Manager for department staff meetings and serves as the recorder of meeting minutes when needed.

CNA License or alternate experience as a caregiver, preferred

An administrative guru with a strong attention to detail and excellent organizational skills.

Very comfortable being on the phone and have strong follow-up skills.

Data entry in various software programs.

Manage the clinic's closing duties.

2+ years of administrative experience.

Previous experience in health care or patient care is preferred

Basic Life Support (BLS) for Health Care Providers certification required.

One year recent experience in a health care setting involving patient contact is preferred.

Exceptional customer services skills and strong desire to work in a fast paced environment

Effective organization and interpersonal communication skills

Informs front end registration staff of registrations and/or authorization protocols by interpreting and communicating insurance rules, regulations, policies, and procedures.

Directly supervises day to day operations of the Patient Logistic/Admitting staff.

Conducts a quality management/improvement/assurance program to ensure compliance with standards for home health services/programs and external regulatory agency requirements.

Five (5) years of supervisory/management experience in a home care setting.

Ensures agency follows the change management process for implementation and development of new forms, policies and procedures.

Displays competency in navigating contracts and working in a Labor/Management Partnership environment.

If additional competency is needed, a plan will be completed upon hire to solidify that experience and knowledge base.
