Health Coach Dietitian Jobs

This is a full-time telework role.

Hours for this position are Monday-Friday 8:30a-5:00p EST.

• Provides subject matter expertise, consultations, case reviews and education to clinical staff and members. • Participates in the inter-disciplinary team and collaborates with healthcare team to align nutritional interventions with care goals. • Evaluates the plan of care and effectiveness/outcomes of dietary interventions and modifies as needed. • Educates members and families on healthy diet, appropriate nutrition guidelines for disease state(s), provides prevention and wellness, guidance. • Connects members with nutritional community resources, dietary counseling services, leveraging applicable nutritional plan benefits. • Collaborates with nutritional therapy providers, home care providers , community resources for nutritional. • Utilizes evidence based knowledge and protocols appropriate to patient populations and diagnoses. • Adheres to care management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. • Other responsibilities as assigned.

Pay Range
The typical pay range for this role is:
Minimum: 45,240
Maximum: 98,280

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications

• 3+ years related work experience as a Registered Dietitian required
• Current Registration as a dietitian with the Commission on Dietetics in state where he/she will work
• Willing and able to obtain licenses in other MCI states that require a license
• 1+ year(s) experience working long-term with people who have chronic conditions e.g. diabetes, renal disease, complex nutritional needs

Preferred Qualifications

• Hospital experience preferred
• Licensed in NC, OH or FL

Education
Masters Degree in Nutrition or related field preferred
