ESSENTIAL FUNCTIONS: Provides comprehensive assessment and diagnosis of behavioral health clients Provides effective treatment planning and assisting clients in successfully achieving goals. Provides assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders. Assists the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease. Evaluates crisis situations and apply appropriate interventions .Assists in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration Work as a member of the Care Team to improve health outcomes. QUALIFICATIONS: Education: Master's Degree in Social Work, Master's Degree in Counseling or PhD in Clinical Psychology required Experience: A minimum of three (3) years of counseling experience required Experience in delivering behavioral health required Knowledge of DSM IV 5 axis diagnosis required Experience in an integrated health care environment preferred Experience in Medicaid, Medicare Advantage, and NCQA Standards preferred Licensure Requirements: Licensed in the State of Missouri as a Licensed Clinical Social Worker (LCSW), License Professional Counselor (LPC) or a Licensed Psychologist. Other: Must have reliable transportation
