In this role, you will play a pivotal role in ensuring the efficient delivery of healthcare services by managing schedules and coordinating care for our patients. Your friendly and professional demeanor will be instrumental in fostering strong relationships between our healthcare providers and those we serve.
Key Responsibilities
Scheduling and Support: Create and manage daily schedules for our Community Health Workers, as they provide invaluable support across hospitals, primary care clinics, and various community organizations.
Healthcare Provider Coordination: Extend your scheduling expertise to coordinate the calendars of healthcare providers, including MDs, Nurse Practitioners, and Licensed Clinical Social Workers.
Efficiency Enhancement: Expertly synthesize and integrate diverse scheduling inputs from the Health Information Exchange (HIE), primary care appointments, and other sources to optimize staff efficiency.
Patient-Centered Approach: Reach out to local hospitals and organizations to gain insight into patient needs pre and post-discharge, ensuring a seamless transition of care.
Treatment Clarity: Develop a deep understanding of Author Health services to ensure timely and adequate support for patients and caregivers.
Patient Engagement: Empower Community Health Workers to establish meaningful connections with patients and caregivers, facilitating behavior change and enhancing the quality of healthcare services.
Provider Collaboration: Coordinate with healthcare providers on medical health records, lab work, appointment scheduling, and pharmacy requests.
Primary Care Partnership: Collaborate with patients' Primary Care Physicians (PCPs), including sending visit notes, monitoring upcoming appointments, and promptly responding to patient inquiries.
Calendar Management: Maintain provider calendars, make patient reminder calls for appointments, and ensure the scheduling of Author Health follow-up visits.
Additional Duties: Perform other tasks as assigned by Behavioral Health Care Managers, contributing to the overall success of our healthcare initiatives.
Requirements
Educational Qualifications: A minimum of a college degree is required, with a Master's Degree being preferred.
Organizational Skills: Demonstrated proficiency in organization and operational management.
Logistics Experience: 2+ years of experience in logistics management for an industry or company.
Information Coordination: Ability to collect, interpret, and synthesize disparate information into a coordinated plan for staff.
Healthcare Insight: Previous healthcare experience, particularly in behavioral health, is advantageous.
Language Skills: Bilingual proficiency in Spanish, English, and Creole is preferred.
