  • This full-time position is a hybrid role that requires working in the office on Tuesdays and Thursdays at 1200 Binz St Suite 1490 Houston TX 77004. Mon, Wed, and Fri are typically work from home but may be in office on occasion.*
The Director, Population Health is responsible for the development, implementation, and ongoing execution for Privia Quality Network within defined ACO Programs and Tactics. This includes collaborating with the VP of Population Health, PQN Leadership team, Tactic Team Leads, other Program / LOB Leads, physician leaders, and clinical leadership to develop and implement value-based programs, performance metrics, accountability, and performance improvement measures as needed. Additionally, this role is responsible for maintaining close relationships with payer partners to ensure Privia is positioned for success given any future program changes.
Essential Job Duties
  • Manage population health team member(s)
  • Identify care center staff/provider education opportunities
  • Collaborate with internal teams to drive success in key areas, including but not limited to Quality, HCC Documentation & Coding, Clinical Operations, and Performance Management
  • Develop strategies for clinical programs and implement and execute programs with providers/care centers
  • Accountable for the program performance, administration, and operations of the MSSP, Medicare Advantage, and other value-based care programs
  • Develop action plans to improve overall performance in the ACO
  • Collaborate with Credentialing team to Manage Value-Based Provider Rosters
  • Collaborate with the National/Regional Population Team, Physician Leaders, and Performance Team to meet performance targets
  • Drive results through dynamic leadership both internally and externally as a customer-facing leader
  • Collaborate with external payer team members and internal team members on strategies to be successful in value-based care programs
  • Plan for and participate in monthly care center meetings and physician meetings to review population health training, initiatives, and data
  • Oversee clinical initiatives progress and analyze the impact on the market
  • Serve as subject matter expert and internal resource for all things related to MSSP, Medicare Advantage, and other value-based programs and collaborate with physician and market leadership to guide program success
  • Meet with potential vendors for clinical programs and work with the Population Health team to identify if a program should be rolled out in the market
Qualifications
  • A proven ability to gain trust and work effectively with physicians
  • Collaborator – able to develop buy-in and drive action across a wide set of team members
  • Experience in value-based payer programs, risk, and successful tactics to improve quality and generate savings required.
  • Strong orientation toward client service, quality, and team building
  • Bachelor’s Degree preferred, MBA or advanced degree highly preferred
  • Demonstrated success in building and maintaining a strong operating organization, reinforcing the mission and culture of an enterprise as exemplified by uniform standards and consistent performance
  • MA/MSSP experience required.
  • Analytical, quantitative, and metrics-driven; knowledgeable on how to interpret value-based program requirements and understand the impact on financial results
  • Strategic – able to develop sophisticated relationships with network partners and payers
  • Comfortable with ambiguity and complexity, and able to move forward despite unclear direction
  • 5+ years of relevant experience as a project or program leader within healthcare
  • Must comply with HIPAA rules and regulations
  • Excellent communicator – expresses complex ideas clearly and effectively to a varied audience of physicians, extenders, care center staff, and Privia employees
  • 5+ years management skills
