Job Summary

Our client is a company that is building a virtual-first, home-based cardiac rehab (CR) solution that provides a comprehensive set of tools to educate, rehabilitate, and empower heart disease patients to live the lives they aspire to live, and in doing so, reduce their lifetime all-cause mortality risk by 50%.

As a Health Coach, you will be responsible for assessing patient cardiovascular health risk factors, developing/managing a personalized care plan tailored to their specific medical, physical, mental, and psychosocial needs, and coordinating their care across our company's and the Health System’s care team. You will be a part of an integrated team of clinical professionals including Nurse Practitioners and Licensed Clinical Social Workers working together to deliver a world class patient experience.

Responsibilities

• Conduct weekly virtual check-ins to discuss patient goals, track progress, and adjust care plans if necessary over the 6-month program
• Prioritize patient safety and escalate issues in real-time for resolution
• Leverage health & wellness coaching techniques (ex. motivational interviewing) to modify patient behavior
• Monitor and leverage remote physiological and activity data to inform patient care
• Engage patients via chat/SMS in between visits to drive motivation
• Develop measurable treatment goals and interventions that are supported by standards of practice and/or current literature
• Evaluate patient functional abilities and limitations to develop customized exercise prescriptions to be performed asynchronously by the patient
• Communicate effectively across our integrated clinical team and our partner health systems
• Provide virtual cardiac recovery services to an individual panel of 50 patients
• Provide complete, accurate and timely documentation of patient sessions within our EHR
• Create personalized care plans that consider the patient’s physical, mental, emotional and behavioral change needs and abilities

Requirements Skills/Traits

• Understanding of relevant industry clinical practice guidelines and quality measures for Cardiac Rehabilitation and Health and Wellness Coaching (ex. AACVPR, ACSM, NB-HWC)
• Experience in cardiac disease management, rehabilitation, and wellness
• Expertise using technology in cardiac rehabilitation (ex. Fitness trackers, remote blood pressure cuffs, weight scales, etc.) or a willingness to learn new technologies
• Comfortable working in a fast-moving and ever-changing startup environment
• Understanding of relevant patient educational content used in Cardiac Rehab
• A willingness to travel at least once per quarter for in-person team meetings
• Effective and empathetic communication skills across phone, video and asynchronous messaging
• An innovative mindset, and willingness to challenge status quo of traditional cardiac rehab experience

Education/Certification/Licenses

• Master's degree or bachelor’s degree with equivalent work experience in Exercise Physiology, Exercise Science, Kinesiology, or a related field at an accredited institution
• Basic Life Support (BLS) certification
• AACVPR Certified Cardiac Rehabilitation Professional Certification or the ACSM Clinical Exercise Physiologist Certification prior to start (and willingness to obtain the other within 6 months of hiring)
• Minimum 5 years of clinical experience
• Wellness Coaching certification from a NBHWC accredited program
