Primary Purpose of Position: The EPSDT/Youth Initiatives Coordinator manages all aspects of the Early and Periodic Screening, Diagnosis, and Treatment process in this position. This position will adhere to the policy for EPSDT. This team member is to perform activities in relation to EPSDT and Youth Initiatives that include but are not limited to identifying eligible members, coordinating outreach and education to members, coordinating with team members for reminders to be sent to members and the tracking of outreach activities. As a part of this role the team member will provide education on the Early and Periodic Screening, Diagnosis, and Treatment Medicaid Program. This position coordinates efforts with other agency staff and with network providers to ensure targeted program outcomes are met.
Role and Responsibilities:
The EPSDT/Youth Initiatives Coordinator is responsible for (though not limited to):
- Establishing/Maintaining Centralized Eligibility List for EPSDT
- Maintaining and managing centralized list for the members that are eligible for EPSDT services.
- Maintain accurate and thorough records of EPSDT eligible members.
- Ensures effective coordination of efforts with staff for managing the EPSDT program.
- Share reports with Care Management to guide the connection of eligible members to services
- Work closely with member and legally responsible person to obtain information needed to determine eligibility as well as specific service needs.
Connecting Individuals with Service
- Encourage all recipients on waiting lists to download and engage with the Pyx Health app for wellness activities and additional support.
- Ensure effective communication with providers to ensure that eligible members receive services.
- Meet bi-monthly to review status of recipients on waiting lists and collaborate to determine if alternative wraparound community services, programs or funding sources are available to assist recipients while they are waiting.
- Send providers appropriate referrals in timely manner when service is available, documenting outcome of screening
- This position will ensure adhere to the EPSDT Services Policy.
- Facilitate members to receive linkage to EPSDT services.
Additionally, the individual in this role will coordinate additional youth initiatives as needed. This role will work with High Fidelity Wrap Around initiatives and Standard Assessment Protocols for youth.
Knowledge, Skills and Abilities:
- Ability to independently organize multiple tasks and priorities and to effectively complete duties within assigned timeframes
- Detail oriented
- Comprehensive knowledge of services which address Mental Health, Intellectual/Developmental Disabilities and Substance Use Disorders.
- Sensitivity and knowledge of different cultures, ethnicities, spiritual beliefs and sexual orientation
- Exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts and established processes
- Ability to manage and uphold integrity and confidentiality of sensitive data
- Excellent computer skills including proficiency in Microsoft Office products (such as Word, Excel, Outlook, etc.)
- Ability to maintain effective and professional relationships with individuals, family members and other members of the care team.
- Must understand and utilize person-first language and a person-first perspective.
- Problem solving, negotiation and conflict resolution skills
- Other duties or task as needed.
- Demonstrated ability to collaborate and communicate effectively in team environment
Education/Experience Required:
Bachelor’s degree with at least two years of progressive experience in the field of mental health, intellectual/developmental disabilities or substance use disorder.
Education/Experience Preferred:
Graduate degree in human services field and at least four years of progressive experience in the field of mental health, intellectual/developmental disabilities or substance use disorder.
Other requirements:
Must reside In North Carolina.
Must have ability to travel.
