Job Description
This position works under direction, provides ongoing support for performance improvement groups and teams, including facilitation and coordination of measurement and improvement activities. Assists with monitoring, implementing and evaluating processes and programs to meet regulatory, accreditation and standards of practice, provides staff with formal and informal education on principals of performance improvement and measurement. Assists Director with various performance improvement projects.


  • Assists the Director, Clinical Excellence with other performance improvement projects as needed. These assignments may include, assisting in planning and implementation of performance improvement related education activities.
  • Develops processes, forms and tools to comply with current regulations and standards.
  • Prepares presentations and reports of project progress and results achieved for various audiences, including Medical Staff Quality, Safety Committee and the Board of Trustees, and prepares communications to inform Linden Oaks Hospital employees and physicians of performance improvement projects, team progress and results achieved.
  • Manages the medical care evaluation (MCE) process ensuring appropriate screening and review of clinical cases, evaluation of care provided, referral to physician MCE process and appropriate clinical and administrative follow up.
  • Reviews policies and procedures for appropriateness and compliance. Reports findings, interprets trends and develops appropriate action plans to correct deficiencies and makes recommendations to Manager.
  • Oversees the development and maintenance of performance improvement programs for Linden Oaks Hospital. Responsible for interpreting trends and developing appropriate action plans. Reports findings and makes recommendation to Director and to the Quality Improvement Committee.
  • Monitors & maintains the Linden Oaks Hospital is compliance with regulatory & accreditation standards, such as coordinating for Joint Commission survey readiness.
  • Oversees data collection/abstraction projects including, but not limited to Scorecard data and assures data quality and integrity through development and implementation of data collection guidelines, reliability checks, and other audit tools.
  • Researches topics related to performance improvement projects and initiatives and stays abreast of trends in performance improvement, patient safety and related areas.


Required Education and/or Experience:
  • Experience facilitating/leading improvement teams and/or projects
  • A Bachelor’s Degree with five or more years of experience in a health care setting


Preferred Education and/or Experience:
  • Formal training and/or certification in Process Improvement and/or Team Facilitation
  • A Master’s Degree in Nursing
  • Experience with Joint Commission survey standards and survey process
  • Experience with CMS/IL Department of Public Health survey standards


Required License and/or Certification:
  • CPI (Crisis Prevention Intervention) certification within 90 days of hire


Preferred License and/or Certification:
  • Certification in quality and/or safety
  • Current state licensure, such as LCPC, LCSW or RN
