Job Description
About The Role


The Clinical Care Manager (CCM) is a core member of a collaborative clinical care team, working alongside our prescribing clinicians (PC), Triage Nurses, and care coordinators. The CCMs support and coordinate the care of approximately 350 patients receiving MOUD during treatment maintenance. CCMs conduct induction check-in calls and urine drug screen visits.


Longitudinal Clinical Assessments: ensure patients remain compliant with essential and recurring clinical tasks, including monthly completion of measurement based care assessments.


Education: Provide patient education on OUD and MOUD.


Monitor/Medication Management: Systematically track treatment response and monitor patients for changes in clinical symptoms and side effects, treatment adherence, effectiveness of treatment or complications; prepare prescription requests for the prescribing clinician.


Behavioral Interventions: use motivational interviewing, behavioral activation and other behavioral interventions as needed to motivate patients to remain in care and adherent to their treatment plan.


Duties and Responsabiities


  • Patient engagement: Facilitate patient engagement and follow-up care in collaboration with primary clinician
  • Treatment Plans: Collaborate with PC to determine individualized treatment plans for patients; facilitate changes to treatment plans when patients are not improving as expected in consultation with the PC
  • Monitor/Medication Management: Systematically track treatment response and monitor patients for changes in clinical symptoms and side effects, treatment adherence, the effectiveness of treatment or complications; prepare prescription requests for the prescribing clinician
  • Provide non-judgmental, non-stigmatizing care to patients with OUD
  • Charting: Document all patient encounters in Ophelia’s EHR in a timely manner
  • Case Consultation: Participate in weekly case consultations
  • Education: Provide patient education on OUD and MOUD
  • Behavioral Interventions: Provide brief behavioral interventions using evidence-based techniques such as problem-solving treatment or motivational interviewing
Training and License Requirements


  • Nurse Practitioner or Physician's Assistant
  • Licensed in PA or NY (both preferred)
Required Skills


  • Basic knowledge of psychopharmacology for opioid use disorder
  • Working knowledge of evidence-based psychosocial treatments, counseling techniques, or brief behavioral intervention
  • Ability to independently manage a large panel of patients
  • Comfortable working with a diverse population of people with opioid use disorder
  • Proficient and comfortable working in a remote telehealth environment
  • Ability to provide patient-centered, non-judgmental, non-stigmatizing compassionate care
  • Working knowledge of differential diagnosis of common substance use disorders
  • Ability to maintain effective and professional relationships with patient and other members of the care team
  • Excited about working in an early-stage healthcare startup
  • Demonstrated experience working in addiction medicine and/or psychiatry (preferred)
  • Demonstrated ability to collaborate and communicate effectively in a team setting
  • 1-3+ years post-training experience
