Case Worker Jobs

A. Provide Case Management to Clients in Accordance with Policy, Procedure and Regulatory Protocol.

• Establish and maintain a network of referral resources
• All other duties as assigned
• Escort clients to appointments as needed
• Serve as a liaison with all other service providers to ensure coordinated delivery of services.
• Collect and supply data and information necessary for monthly reports to the Administrator or regulatory agencies.

• Enabling service users to access education and training opportunities
• Giving service users access to medical and healthcare services
• Working with public and private organizations that provide services
• Empathy and the ability to identify service user’ areas of need
• Excellent time management and organizational skills and the ability to manage their workload
• Excellent communication skills and the ability to impart information in a Way that is accessible to their service users
• Strong problem solving skills
• Excellent critical thinking skills
• The ability to write and deliver accurate and thorough reports
• Resilience and the ability to maintain a professional approach even in difficult circumstances
• Application of crisis intervention techniques and methods
