Summary Of Duties

The Case Manager is primarily responsible for providing case management to clients as the lead of a service team. Works in partnership to meet the client needs, linkage to community supports and plan for discharge, including aftercare. Primarily responsible for the Assessment and development of the service plan, will coordinate with other team members to meet client needs. Provide evidence based interventions as described in program policy and procedures.

Education And Training Requirements

• Four-Year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing.

Experience

• Experience with servicing seniors aged 60+, and their caregivers, with long term case management and wellness activities.
• Over two years, up to and including four years.

Special Knowledge, Skills, And Abilities Required

• Knowledge in the area of gerontology and case management.

• This job requires a valid Hawaii driver's license and a clear driver's abstract and continued employment may be contingent on maintaining a clear driver's abstract.
• This job requires a valid Hawaii driver's license, a clear driver's abstract, and use of personal vehicle to transport clients, as needed, as well as a willingness to travel in personal vehicle on an as-needed basis throughout the island.
