Job Title: Case Manager
Department: Youth Crisis & Career Services
Reports to: YCCS Program Manager
Status: Non-Exempt, Full-Time
Position Summary:
The YCCS Case Manager position requires a team member that can create and maintain positive consistent contact with youth ages 14-24 who participate in YCCS programs. This position will focus on providing wrap-around comprehensive assistance to youth in the areas of housing, employment and life skill building. The position requires the worker to be comfortable communicating and presenting to youth and community partners.
Minimum Qualifications:
- Competence in working with cultural and socioeconomic diverse clients
- Must pass criminal background check and TB screening
- Must have physical ability to actively complete all job duties
- Experience working with youth (preferred)
Essential Functions and Job Responsibilities:
Youth Engagement
- Establish and maintain positive relationships with youth
- Help orient participants in program expectations
- Collaborate with community partners to maximize assistance to youth
- Provide community referrals for health, housing and employment services
- Advocate on behalf of youth to get access to services and basic needs
- Assist youth in setting goals, creating case plans and advocating on the clients behalf
- Ability to access and problem solve high stress situations
Employment
- Educate and assist youth on employment opportunities and eligibility
- Assist in documentation collection and resume building
- Identify opportunities for career development, employment or volunteering
- Create workshops for job readiness, work experience, trainings and skill building
- Maintain compliance when working with state/ federal guidelines
Housing
- Ability to facilitate access to community resources as necessary
- Locate appropriate housing for youth guided by participant choice
- Responsible for increasing income and assisting youth to achieve their housing goals
- Assess needs regularly to ensure successful transition into independence
- Advocate on youths behalf to stabilize their housing situations
Community Relations
- Participate on boards, groups, tabling's and other community sharing engagements
- Maintain comprehensive data, notes and follow up plans on various confidential platforms
- Create relationships with local businesses, organizations and service clubs
Administrative
- Comply with the organizations safety protocols and procedures
- Demonstrate a high level of professionalism and appropriate boundaries
- Ability to balance a case load and maintain organized files
- Collaborate with team to reviews case plans, program exits and follow up
Miscellaneous
- Work evening or weekend hours as required
- Perform other related duties as assigned
- Participate in training and supervision
Due to the leadership role SAY holds in the community, your supervisor will ask you to assume responsibilities and tasks during times of crisis that may be outside your normal duties.
Knowledge, Skills and Abilities:
- Stay up to date on current trends impacting youth in the community
- Availability to have a flexible work schedule
- Knowledge of challenges which may affect homeless transitional aged youth including: mental health, poverty, life skills development etc.
- Demonstrated strong communication skills, both written and verbal
- Maintain strict confidentiality and boundaries related to clients and other agencies
- Ability to work independently with minimal direction
- Knowledge on sub-cultures & positive youth development
Bonus Requirements:
- Any second Language (preferred)
