They work with families to ensure that they have a safe and permanent home. They also provide support to foster parents and other professionals involved in the foster care system. Permanency Case Managers work with families to develop plans for reunification, adoption, guardianship, or other permanency options. They also provide support to families and children during the transition to a permanent home. Permanency Case Managers are responsible for conducting assessments of families and children in foster care. They assess the needs of the family and child, and develop an individualized plan for permanency. They also provide support to families and children during the transition to a permanent home. Permanency Case Managers are responsible for advocating for the needs of families and children in foster care. They work with other professionals in the foster care system to ensure that the needs of the family and child are met.
