Employment Case Manager Jobs

PURPOSE: The primary function of this position is to identify the barriers to employment, education, physical or mental health services and connection to community systems. Position will create individualized plan to overcome barriers through an assessment, training, education and connection to resources.
Ability to create linkages for supportive services with employers and business partners is critical.
KEY RESULT AREAS: Facilitate assessment to identify barriers related to transportation, employment, education, housing and wellness (physical and/or mental health). Create individualized plans to identify services and interventions to address barriers identified in the assessment. Create linkages to resources identified in plan. Provide transportation, support and education to various community appointments.
Facilitate life skill groups to address topics including, but not limited to, employability, education and career planning, financial literacy, emergency and safety, legal and health. Recruit and train mentors or volunteers interested in serving disconnected youth ages 18-24 Adhere to documentation standards including intake, assessment, case notes and mont hly reporting. TRAINING AND EDUCATION : Ability to facilitate three 10 week sessions of Employment Training during non traditional hours including evenings and weekends if necessary. Utilize Vita Nova Thrift Store and On the Job Training for identified participants without any previous work experience or who may have a troubled employment history. Weekly contact with program participants.
REQUIREMENTS: Bachelor's Degree in Social Services (Masters preferred). Ability to work non-traditional hours including evenings and weekends. Creative / innovative does not easily give up.
