Job Description
Job Title: Case Manager
Reports to: Program Manager
Status: Full-time
Division/Department: Workforce Development
Salary Range: $38,500 – $42,500

OIC of South Florida is a community-based leader in providing quality self-help skills and employment opportunities for disadvantaged and underemployed residents; thus, assisting them to become self-sufficient and productive members of society. Its work is accomplished through four divisions: Workforce Development, Youth and Families, the OIC Workforce Academy, and OIC Strategic Integration).
Position Summary:
The Case Manager assists clients in breaking the cycle of recidivism by providing full wraparound case management and employment services with a heightened focus on self-sufficiency. The Case Manager facilitates all service and supportive service referrals based on the unique needs of each program participant through outreach with Florida Department of Corrections and community providers. The Case Manager works with a multi-tiered support system to help participants develop a comprehensive plan and work toward attaining their career pathway goals. The Case Manager follows up on all service referrals and collaboratively works with the participants to ensure they can successfully transition out of the prison system.
Essential Duties and Responsibilities:
Provide services that address criminogenic and basic needs for participants. Deliver services to participants working to meet goals identified in their case plans
Coordinate and work with participants to identify a career ladder/pathway and training/education opportunities to move the participant along their chosen career path and family sustaining employment.
Refer participants to partner agencies for which OIC does not provide direct service delivery and provide monthly follow-up
Coordinates the participant’s wrap-around case planning and facilitates case planning meetings with support team.
Monitors and tracks case plan progress, working with support team members to document goals achieved on a monthly basis
Provide Project Director with a monthly update regarding the process of clients on the assigned case load
Maintain contact with participants based upon their risk/need and provide appropriate counseling services
Build and maintain professional working relationships with other team members, WRC and prison personnel, community service providers, employers etc.
Attend scheduled meetings deemed necessary by the Project Manager or other Senior Management Staff to include community meetings and in-services.
Follow all requirements and guidelines of program funder under guidance of supervisors
Collect and input data into program evaluation database(s) for ongoing supervisory and funder review
Other duties as assigned and needed for program support.
Education and/or Work Experience Requirements:
Bachelor’s Degree in Social Work or related field. CCM, LCSW, SSW, MSW certification or degrees are a plus
Minimum of three to five (3-5) years relevant experience in case management, mentoring, and support to at- risk population. Experience in working with at-risk populations returning from incarceration preferred
Ability to establish working relationships with co-workers and function as part of a cohesive team
Ability to develop cross-agency partnerships with educators, business and industry partners, and government agencies
Demonstrates current knowledge of and the ability to apply instructional technology applications that support the diverse learning needs of students
Ability to express complex ideas with clarity and precision both orally and written
Strong interpersonal skills
Ability to handle multiple deadlines and tasks simultaneously
Effective organizational planning and project management skill
Ability to effectively work and communicate with clients, postsecondary personnel, and community partners from diverse cultures or backgrounds in
Maintain integrity of confidential information relating to clients, staff, or stakeholders
Bilingual (English/Spanish) preferred
Other Requirements:

Knowledge of methods for job development, local labor market trends & employment opportunities.
Ability to build and maintain relationships and partnerships; maintain confidentiality, professionalism and maturity in handling sensitive information, participant interaction and community relations.
Experience providing training and mentoring.
Ability to obtain FDLE/FDC clearance to enter correctional facilities
Experience in work force development preferred.
Experience working with at-risk populations providing counseling and assessment is a plus.
Ability to drive long distances on a weekly basis to meet with incarcerated participants
Strong leadership, organizational and analytical skills.
Ability to organize, plan, and manage multiple tasks.
Ability to manage time efficiently and meet deadlines.
Excellent interpersonal skills; a collaborative, team-oriented approach.
Ability to work with and engage individuals of diverse backgrounds.
Exceptional written and oral communication skills.
Must possess a proficiency working with Microsoft Office Suite and the ability to successfully navigate through the Internet, excel, power point, and other applicable software.
This position will require independent travel between agency resource centers and other venues as required by position. Must maintain valid DL and insurance coverage on personal vehicle at all times during employment.
