Sword is looking for an experienced manager to expand our Sword’s Care Integration team. This role is a part of the broader Customer Success organization and is responsible for building deep and strategic integrations between Sword Health and our customers’ benefits ecosystems. This role is cross-functional; you’ll work directly with Customer Success, Clinical, Product teams, and most strategic partners to implement scalable programs that facilitate inbound and outbound referrals, drive utilization of Sword, and ensure our members are receiving the appropriate care that they need. It requires both internal relationship building and external relationship management; this role works with internal teams as well as customers and their vendors.


What You'll Do


  • Experience working with the benefits space, healthcare, and/or clinical background a plus
  • Work with our customers and their partners to integrate Sword within their health and wellness ecosystems
  • Develop deep understanding of utilization and transfer best practices in the healthcare space, especially between providers, care navigators, COEs, EMOs, etc
  • Work with product and data teams to measure, refine, and scale these care integration processes
  • Create and manage documentation, develop needed management systems and protocol, and support technologies that improve the scalability of our process
  • Assist integrating into customer ecosystems throughout the continuum of care and at the appropriate time for the member
ABOUT YOU


  • Exceptional written and verbal communication skills, strong presentation and public speaking skills, and ability to create and maintain process documentation
  • Highly collaborative, goal-oriented, detail oriented, and creative
  • Ability to operate strategically and independently in a fast-paced and rapidly evolving environment
  • Passionate about health equity, social determinants of health, and bringing world-class healthcare to those who need it
  • Ability to work directly with multiple internal teams and external partners to build, implement, and optimize inbound and outbound referral processes
  • 5 or more years of Customer Success/Implementation, Care/Disease Management, Program Management, Benefits, Strategic Partnerships or Operations experience, preferably in healthcare; clinical background is a plus
  • Experience building and managing highly effective operational processes at scale
