Strong time management, organizational and planning skills.

Help the patient to put systems in place in their own environment to assist with the management of their care.

Refer to internal or external case management services when other issues are identified (i.e. hunger issues, domestic violence issues, etc.)

Provides patient education that is cultural and linguistically appropriate

Provide advocacy, patient education and support in accessing community-based and hospital-based programs.

Minimum two years of working experience. Previous work in community settings preferred.

Knowledge of sterilization standards/procedures and associated terminology.

Maintains clean, safe and organized workspace and other associated tasks.

Inspects for cleanliness and proper functioning, assembles, packages and records instruments/devices and equipment following the established procedure for each.

Performs, monitors and records routine safety/efficacy testing of associated equipment in accordance with manufacturer's recommendations and industry standards.

Completes Basic Life Support (BLS) education every two years

Knowledge of basic nursing procedures.

Assists in patient assessment according to standard procedure and needs of patient and their families.

Administers medications, performs treatment therapies, and operates equipment according to policy and procedure.

Assists in implementation and evaluation of care plan including long and short-term goals, nursing actions and teaching.

Cares for patients with peripheral access devices
