Rn Hospice Home Care Jobs in Utah

Provides skilled observation, nursing assessment, development and implementation of appropriate home care treatment plans for participants with home care needs.

Collaborates with other supervisors/managers and department staff though effective communication, demonstrated leadership and facilitation of change opportunities.

Physical requirements include ability to lift/move up to 75 pounds occasionally and up to 50 pounds frequently.

Two+ years' experience as an RN working with the geriatric population preferred

Two+ years' experience as an RN working in home care and or home health care preferred

Leadership or supervisory experience preferred

Conducts special clinical studies related to patient care management.

Must be able to work in a Labor/Management Partnership environment.

Detailed knowledge of Title 22, TJC, OASIS and NCQA requirements for licensure and accreditation.

Monitors compliance with quality improvement requirements.

Minimum two (2) years of experience as a licensed registered nurse.

Minimum two (2) years of experience in one of the following areas: Healthcare Quality, Accreditation, Consulting or Strategic Planning.
