Provide medication management for clients

A current Massachusetts RN/LPN license with at least 2 years nursing experience

Strong clinical and computer skills

The ability to prioritize and demonstrate problem solving skills.

Previous home care experience is preferred; Medicare/Certified service experience is not required.

Proven selling and customer relationship management skills with the ability to navigate a sales process to include stakeholders/partners.

Bachelor's in Business and/or equivalent combination of education and experience.

Estimated salary range $42859 - $93500 / year. Actual salary will vary by geographic location and experience.

No on-call requirement. Flexible schedules.

Document timely, accurately, and at the point of care using a tablet, including OASIS assessments, SOC/Admissions, recertification’s, physician orders, etc.

One (1+) year of clinical experience as a Registered Nurse (RN) required

Intuitive knowledge of technology and comfortable with smartphones, tablets, virtual reality, interactive software and artificial intelligence

Supervise caseload including LPN’s and Home Health Aides. Facilitate care coordination with PRN staff when care is provided for caseload.

Care Collaboration/Coordination with primary and specialty healthcare providers.

Coordinates monthly and ongoing education requirements for Home Health Aides.

Assists with colleague clinical education needs and observation of clinical practice as requested by Clinical Manager or other appropriate leadership.

Collaborates with the manager to develop an education plan regarding clinical documentation issues not in keeping with hospice standards, when applicable.

Collaborates with orientee, information technology, preceptor, clinical manager, scheduler to develop and implement an individualized orientation schedule and learning plan.

Responsible for coordination and maintenance of education record keeping.

Represents education at weekly process meetings and monthly team meetings for home care and/or hospice.

Trained Volunteers- offer companionship for patients and respite for families

Training: Our new staff will feel confident knowing they have learned from experienced preceptors within their area of specialty.

Home care experience as a Certified Nursing Assistant

Experience in a supervised health care setting, such as a hospital or nursing home.

Current Healthcare Provider BLS certification by date of hire
