Family Service Advocate Jobs

Provides family focused, solution-based risk assessments of families and conducts home studies.

Initiates alternative out-of-home placements such as in the home of a relative or in foster care for children needing care.

Completes individual service plans and group care packets, as needed.

Writes and reviews progress reports and status change reports. Document all contact and services.

Monitors visits between children and their families when appropriate. Ensure monthly contact with placement families and individually with the children.

Diligently pursues active efforts with parents to ensure rapid and viable reunification.

Builds trusting relationships with parents to provide professional case management, prevention and early intervention services.

PRINCIPAL DUTIES AND RESPONSIBILITIES(Not All Inclusive)

Monitors child's attendance and works closely with parents to identify obstacles and problem solve with them to establish regular attendance.

Actively promotes Head Start in the community by participating in recruitment efforts and activities.

Supports parents with transition into and out of ELE services

Demonstrates understanding of the child abuse/neglect laws and reports suspected child abuse to the Family & Community Services Director.

Ability to interpret verbal and non-verbal communication; demonstrate situational awareness, compassion and empathetic listening skills

Previous experience effectively advocating, collaborating and negotiating with medical professionals to solve complex problems

Knowledge / Skills / Abilities:

Minimum Educational Requirements Associate Degree

Provide materials, activities and training to parents and families to help them effectively support their children at home and at school

Help parents and families to understand curriculum standards and assessments.

Participates in the school's School Leadership Team (SLT)

Organizes a flexible number of curriculum-based programs and activities for family engagement (quarterly)

Knowledge of best practices in delivery quality early care and education with family engagement

Knowledge of working with children and families through a multi-generational approach

Experience providing services to children and families in diverse communities

Recruitment, intake and enrollment, eligibility and attendance monitoring

Support collection and maintenance health and immunization records

Develop and conduct parent workshops

Read current literature to maintain and develop knowledge of issues affecting families and service and in the delivery of care

3 years' experience in a health care clinical setting required

Coordinate employee training on patient/family complaints and grievances

Identify areas needing additional training in policies and procedures related to complaints and grievances

Ensure that issues and concerns raised by patients and their families are handled in an effective and expedient manner

Assume on-call duties after hours, on weekends, and on holidays on a rotating basis

Previous case management experience working with children and families

Conduct crisis case management and intensive case management, as appropriate

Assist clients in completing applications for Texas Crime Victim’s Compensation Benefits

Possess a working knowledge of child abuse issues

Salary range: 47,000 plus depending on years of experience

Provide consistent follow-up with clients, often as a liaison between families and partner agencies

Maintains effective communication with essential departments within the agency: Quality, HR, Education, Development, Finance and Community.

Responds to referrals, on site or remotely, as defined in LOPA policy and procedures.

Attends required department meetings, educational opportunities and other relevant meetings.

Participates in hospital staff and community education.

Use of fine motor skills.

Call requirements: 288 hours per month (12 days, 24 hour shifts)

At least 2 years documented work experience in early childhood development, family development, social work, human services, and/or psychology required.

Recruit and enroll children that are income eligible for Head Start services.

Provide weekly home visits with parents/guardians using the Parents as Teachers curriculum.

Conduct developmental screenings and assessments for children assigned to caseload.

Ensure children receive required health screenings and follow up on concerns.

Provide community resources for families relevant to their needs.

Utilization management intake or status

Timely management of assigned queue tasks requiring research and resolution

Excellent multi-channel communication and problem-solving skills

Ability to communicate effectively in both voice and chat channels, including critical thinking skills, effective typing, grammar, and spelling skills.

Customer service experience within a healthcare contact center, provider office, or healthcare institution

Experience in successful resolution of high level/sensitive customer service issues.

Builds trusting relationships with parents to provide professional case management, prevention and early intervention services.

Ability to make effective and independent decisions. Ability to multitask, prioritize and effectively adapt to a fast paced changing environment.

FSWs are responsible case management of 14-18 cases

FSWs will ensure that all medical, dental, and educational needs are met and are responsible for maintaining child's records.

Documentation that is on-line/ web-based and available to you from home

$50000 - $58000 / year based on education and clinical license

FSWs serve as a youth's guardian and must be aware of DCS policies that guide care and services.

Specialists meet with supervisors at least three times per week to discuss progress and personal development.

Computer experience with knowledge of Microsoft Word, Microsoft Excel, Outlook, and Google Suite.

Prior experience working in an ECEAP setting.

Computer experience, specifically working with the Google Suite.

Bachelor’s Degree in Social Services, Human Services, or Education preferred.

Experience providing Social Services to Children and Families preferred.

Demonstrated accurate recordkeeping skills with a clear organization system.

Determines eligibility for various social service programs offered by the agency or available in the community.

Knowledge of interviewing to obtain facts, explore issues and identify courses of action.

Follow and implement all Head Start/Early Head Start/Voluntary Preschool standards, scopes of services, state licensing to ensure compliance.
