Pop Health Advocate - CMA (FT) BRMMC - Patient Centered Medical Home

Full-time (scheduled 72 hrs or more per pay period)

8 AM to 5 PM

Support parents to increase their knowledge of child development and strengthen parenting skills.

2 years of relevant work experience required

Previous case management experience working with children and families

Conduct crisis case management and intensive case management, as appropriate

Assist clients in completing applications for Texas Crime Victim’s Compensation Benefits

Possess a working knowledge of child abuse issues

Salary range: 47,000 plus depending on years of experience

Provide consistent follow-up with clients, often as a liaison between families and partner agencies

Preferred Experience, Education & Skills:

Build individual and family capacity by increasing health knowledge and self-sufficiency.

3 years’ experience working in health care or working with vulnerable populations.

Strong verbal and written communication skills

Strong customer service, phone, and computer skills

Referring members to Clinical Advocates (Nurse) for conditions that require clinical care and case management assistance
