Since 1970, Hale Kipa has championed Hawai'i's youth and children. We provide youth outreach, independent living facilities, therapeutic foster care, a haven for runaways, and more. Our founders recognized a need for a nonprofit to step in to assist this deserving population and we are still doing that critical work today, on O'ahu, Kaua'i, and the Island of Hawai'i. Learn more about Hale Kipa's programs . Role Overview As a Wraparound Community Navigator at Hale Kipa, your role will be integral to the success of our Wraparound Program. Working alongside the Wraparound Facilitator/Care Coordinator and the entire Wraparound Team, you will provide services to youth transitioning from the Hawaii Youth Correctional Facility. Our goal is to equip these young individuals and their families with the services identified in their Plan of Care, promoting successful reintegration into the community and providing them with the necessary resources and tools to become positive contributors to society. Your responsibilities will encompass engaging with the youth and their families, building relationships, offering robust support, and participating in all Wraparound meetings. You will work in concert with all service providers involved in the Youth's Plan of Care, holding everyone accountable for the participation and completion of identified tasks. A thorough understanding of Hawaii's System of Care is required, including familiarity with agencies such as the Department of Human Services, Family Court, the Department of Education, and others. You will also be expected to connect youth and families with appropriate community resources, network with all relevant case contacts, social institutions, and organizations that provide services for the youth and families. Providing transportation for the youth to appointments and other designated activities is another significant part of the role. Effective, clear, and timely communication with the youth, their families, and all other workers/agencies connected to the youth is imperative. Additionally, proficiency in computer skills, the ability to navigate our electronic health record, and submitting required documentation promptly are also essential aspects of this position. Qualifications Bachelor's degree in a related field such as Social Work, Psychology, Criminal Justice or Human Services, accompanied by experience with youth; OR a minimum of 5 years of experience working with youth.
