Job Description
Position Summary:
The Behavioral Health MSW Group Facilitator is accountable for offering direct services and organizing care for clients with behavioral disturbances, especially those with complex needs. They are supervised by the Program Manager, with oversight from the Director of Behavioral Health Operations. The Group Facilitator guarantees that clients are engaged in acculturation groups and prosocial activities, and that group participants also receive essential care, referral services, and access to maintain optimal functionality. The facilitator attends case conferences, treatment team meetings, and presentations, follows San Ysidro Health policies and program funding contract requirements, and provides crisis intervention services when necessary.
Essential Functions of the Job:
  • Develops relevant guides and resources for acculturational education, lessons, and outreach to schools and parents in English and Afghan language(s) for new arrivals (Pashto, Farsi, Dari)
  • Conducts comprehensive assessment of the Client's mental health and psychosocial needs, including mental health literacy status and deficits, and develops a plan in collaboration with the client and family or caregiver in conjunction with Behavioral Health Clinicians and Primary Care Providers.
  • Documents, identifies, defines, and mitigates any barriers to outreach and group participation.
  • Provides direct client services, including facilitation of psychoeducational groups and case management for group participants.
  • Provides outreach and collaborates with local schools and other community agencies to identify eligible clients for program services and facilitates the referral process.
  • Meets productivity standard for group and client visits according to contract guidelines.
  • Maintains timely, complete, and accurate records with detailed assessment results, action plans, and progress notes.
  • Assists in preparing weekly and monthly narrative and statistical reports covering progress toward meeting outcome and process objectives established in the Behavioral Health grant proposal and as required by various federal, state, and local programs.
  • Educates clients, their families or caregivers, and members of the health care delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, case management, etc., to make timely and informed decisions. Refers Clients with social and emotional problems to other community agencies for assistance.
  • Collaborates with schools’ counselors and district coordinator to provide acculturation groups at elementary, middle, and high schools.
  • Maintains close contact with clients throughout their care. Identifies the reason for missed appointments and groups and assists clients in maintaining continuity of care with appropriate interventions.
  • Documents plans discussed with the patient, family, Behavioral Health Clinicians, PCPs, etc., in the electronic health record system to ensure communication is available to all team members.
  • Supports development of the therapeutic alliance with clients by providing orientation to program services.
Additional Duties and Responsibilities:
  • Enhances professional growth and development through participation in educational programs, case conferences or treatment team presentations, current literature, in-service meetings, and workshops.
  • Attends meetings as required and participates on committees as directed.
  • Performs other related duties as assigned or requested
  • Complies with established departmental and health center-wide policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.

Job Requirements:
Education Required (Minimum level of Education): Master’s degree in social work or marriage and family therapy
Certifications/License Required: Registration as ASW or AMFT with the Board of Behavioral Sciences within 90 days of hire; CA drivers license with appropriate insurance coverage
Experience Required: Completion of practicum/internship as part of master’s degree program. Preferred: 1 year of experience in a non-profit organization.
Verbal and Written Skills Required to Perform the Job:
Bilingual in English and Farsi and/or Pashto. Strong written and verbal communication skills in all languages.
Technical Knowledge and Skills Required to Perform the Job:
  • Must possess excellent customer service, communication and documentation skills.
  • Knowledge of Afghan culture and ability to create culturally tailored products and services.
  • Strong computer skills including experience and knowledge with Microsoft Office software.
  • Knowledge of and adherence to HIPAA requirements.
